Wider Circle is hiring an Associate Care Advocate to join our Care Delivery team. This is an entry-level job with a clear path to move into a more senior role. In this job, you will call patients by phone to learn what challenges affect their health. These may include things like trouble getting food, finding housing, or getting to medical appointments. You will help connect them with services and keep clear, accurate notes about each call. We are looking for someone who is kind, organized, and excited to help people in their community. What You Will Do Screen Patients and Manage Intake You will call patients and ask health-related questions using a set script. These calls usually last 15 to 20 minutes. Your goal is to make patients feel comfortable so they share honest answers. Connect Patients to Resources You will ask questions to learn what each patient needs. Then you will share resources and send referrals to health and mental health services. Keep Accurate Records During each call, you will type patient responses into a health records system called an Electronic Health Record (EHR). You will review and finalize your notes before the end of each workday. All records must follow federal privacy rules (HIPAA) and billing standards. You will also collect and record patient consent at the start of each call. Manage Your Schedule You will keep your own daily calendar, send reminder calls to reduce missed appointments, and meet weekly goals. You will also schedule follow-up calls for patients and leave time between appointments to finish your notes. Requirements • Education: A high school diploma or GED is required. A degree in Human Services, Public Health, or a related field is a plus. • Languages: Speaking both English and Spanish (or another language) is strongly preferred. • Certification: A Community Health Worker (CHW) certificate is preferred. If you do not have one, you must be willing to earn it by attending training provided by Wider Circle and/or completing a state certification application. • Experience: Experience in healthcare, social services, or community work is helpful but not required. You must feel comfortable speaking with many different types of people by phone. We provide full training. • Tech Skills: Comfortable using websites and apps to enter and track data. Able to adapt in a fast-moving work environment. • Self-Directed: Able to manage your own tasks, stay on top of notes, and meet goals without close supervision. • Training: Willing to complete our onboarding program, which includes call practice, documentation exercises, and role-play. You must pass a final test with a score of 80% or higher. • Background Check: Willing to complete a drug test and background check before starting. • You must be able to sit, stand, and walk as needed. You must see and hear well enough to speak with patients by phone and in person. You must be able to type and use multiple computer programs at once while talking. • You will need a strong internet connection and a quiet, private space for patient calls. Gameday Monitor, MLB MLB is currently seeking part-time seasonal Gameday Monitors for the upcoming 2026 Major League Baseball season to provide quality assurance for live video and audio streams on MLB.TV. Ideal candidates will be available to work a minimum of 25 hours per week and mustpossess MLB's core competencies in areas such as collaboration, communication, critical thinking and relationship management. Please note, Gameday Monitors will work remotely and must have a keen attention to detail, strong writing skills and will need to be actively engaged with their teammates during each shift.
