Managing a high volume of patient interactions, the full-time LVN/LPN Night Shift will operate within a centralized clinical call center, conducting telephonic check-ins, triaging alerts, and providing ongoing patient education while working remotely.Key ResponsibilitiesManage inbound and outbound telephonic patient interactions, with an expected call volume of 70 to 90 calls per dayMonitor and respond to Remote Patient Monitoring alerts in real time, escalating concerns as necessaryDocument all patient interactions accurately in the clinical platform while meeting performance metrics related to care quality and engagementRequired QualificationsActive compact LVN/LPN license requiredProficient with computers, EMRs, and telehealth toolsDemonstrated AI fluency, actively using tools like Claude or ChatGPT in daily workAbility to work in a fast-paced, high-volume call center environmentComfort with performance-based metrics and accountability to targets
