Adult Case Manager | Certified Mental Health Specialist Join a compassionate team delivering interactive mental health services to adults and children facing significant mental health challenges. The primary goal is to enhance overall functioning and support individuals in maintaining their lives within the least restrictive environments of their homes and communities. Essential Responsibilities • Provide skill training focused on target goals with measurable outcomes, as determined by a Qualified Mental Health Professional. • Deliver intensive, community-based mental health services using a strengths-based, skill-building approach. • Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans. • Help clients understand their symptoms and explore possible solutions. • Assist clients and families in engaging natural community supports to develop effective solutions. • Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners. • Adhere to privacy and security regulations regarding Protected Health Information (PHI). • Maintain professional and effective communication with clients, team members, and external agencies. Education and Experience • Required: Bachelor's degree in Social Work, Psychology, or related field; or Associate's degree plus 2+ years of experience educating clients on self-care and daily living activities; or equivalent combination totaling 4+ years. • Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics. • Preferred: 3-5 years of professional experience in mental health services, crisis response, or related fields. Licenses and Certifications • Completion of Adult Case Management training within 60 days of employment. • CPST Case Management training, KHS training, and passing background checks including KBI, child and adult abuse registries, and motor vehicle screening. • Valid driver's license, insurance, and reliable vehicle if driving is required. Skills and Abilities • Familiarity with community mental health services. • Strong organizational skills and intermediate proficiency with Microsoft Office. Physical Requirements Light work involving occasional exertion up to 25 lbs and frequent exertion up to 10 lbs. Standing or walking may be required 11-25% of the day.
